Due to changes in the GP Contract, groups of practices are now working together - typically covering between 30-50 thousand patients in an area.
Primary Care Networks build on a core of current primary care services and enable greater provison of proactive, persoanlised, coordinated and more integrated health and social care services.
Primary Care Networks provide the basis for future collaboration with other providers (eg Community Trusts) where appropriater, and will work together to deliver specific packages of care.
